One of the best examples I know of is the Playa Vista project in LA.  There's a description in a case study I did for the CIWMB (see http://www.ciwmb.ca.gov/LGLibrary/Innovations/BizRecycle/CaseStudies.htm).  Also, they will be speaking in the session I organized at the CRRA in Oakland, CA on 7/16 on "Zero Waste Businesses," as they accomplished >90% diversion from the initial demolition efforts in preparing their site for development, in response to the permit requirement.

Has anyone been able to get recycling requirements included in conditions of approval for a use permit?  Our Planning Dept. is looking at a conditional use permit for a motorcycle dismantler.  Planning would like to require recycling (not just proper disposal) of automotive fluids, solvents, and metals, and they asked me for language to put into the permit.  Planning would like to avoid fuzzy requirements like "recycle to the extent practicable" -- they want something with teeth.  Any suggestions?
